Your physiotherapist has shown you the exercises. When it is time to do them at home, you want the instructions ready and a way to see which sessions you completed.
If your physiotherapist uses Forma, they can prepare the plan and share it with you. Open the assigned routine when you are ready to exercise. You can also add the plan yourself.
Get your plan from your physiotherapist
If your physiotherapist uses Forma, they can choose the exercises, add instructions and assign your plan with a schedule.
Open their invitation link and sign in or create an account to connect. Once they assign the plan, it appears in Forma with the exercise details already filled in.
Add the plan yourself if you need to
If your physiotherapist does not use Forma, create a personal routine and give it a name you recognise, such as “Home exercises”.
Add exercises from your plan in the order you were given. Choose a library exercise if it matches the movement from your appointment, or create your own exercise with its own name and instructions.
For each exercise, enter the details from your plan:
- Sets and repetitions, or how long to hold the position.
- Rest between sets, if specified.
- Notes about the equipment, resistance and which side to exercise.
- Your physiotherapist’s cues, such as where to place a band or what to hold.
Keep the demo with the exercise
Open an exercise’s guidance before starting if you need a reminder. If your physiotherapist sent you a video link, add it to your custom exercise in Forma.
Keep useful setup cues with the exercise. For example: “Attach the band at the point shown during the appointment.” Your physiotherapist can add these notes to an assigned plan, or you can add them to your own routine.
Check that any video matches your prescribed exercise. If you are unsure about the movement or how much to do, ask your physiotherapist. Follow their instructions. [1]
Open the routine and follow the session
When it is time to exercise, open your routine and start the session. Forma takes you through the exercises and shows the sets, repetitions or time in the plan.
Open the exercise notes or demo when you need a reminder. Mark the work complete as you finish it. If you stop partway through, leave the remaining exercises unfinished.
For example, a routine with three exercises can keep each movement’s instructions beside its own targets. You move through the plan without finding each instruction again.
Check completed sessions in Activity
Open Activity to see the days when you completed exercises or a full routine. Select a day to see the routine and the number of exercises recorded as complete.
Before your next appointment, use this history to check which days you exercised and whether you finished the routine. You can show the record to your physiotherapist when discussing how the plan went.
If your physiotherapist assigned the plan through Forma, they can also review completed sessions in their dashboard.
Activity records completion. It does not assess your technique or measure recovery.
Keep following the current plan
For an assigned plan, your physiotherapist manages the exercises and instructions. Ask them to update the plan when your recommendations change.
If you created a personal routine, update the relevant exercise with the new repetitions, time or instructions from your physiotherapist. Check the details before your next session.
Keep following the frequency and resistance your physiotherapist prescribed. Ask them about unclear details rather than choosing new targets yourself. [1]
